Here, we can post some difficult things we have done in video games. It doesn't actually have to be considered difficult by many. It can simply be something you're proud of or just liked the process of attaining.
Below are minor spoilers for the very endgame of Hollow Knight:

Probably my best achievement is beating DOOM (2016) and DOOM Eternal on Nightmare. For the original Doom and Doom II I've only done Ultra-Violence, but those games' Nightmare difficulty is kind of insane with the respawning monsters. I know people have quite mixed feelings on the direction Eternal took, but I personally really liked it. The arcade-y feeling of the interface and the pickups spurting out of enemies as you kill them and all the jumping around just scratched an itch for me.

Though they aren't particularly impressive compared to most, these are probably the hardest things I've done:
Touhou Fuujinroku ~ Mountain of Faith (2007): ExNB (cleared Extra with no bombs). I still died a lot on the two timeout spells. But hey, it's a start.
Armed Police Batrider (1997): Advanced course 1cc with Maria. Probably the hardest STG I've beaten. Though a Lunatic 1cc of Touhou Chireiden ~ Subterranean Animism (2008) with ReimuB comes close. I'm working on one of Undefined Fantastic Object and that might beat even APB when I finish it.
Myst (1993) and Riven (1997): Beat with no walkthroughs. It was more of a collaborative effort with me and two IRL friends. But Riven especially is extremely obtuse and lateral at times.
Heroes of Might and Magic III (1999): Finished The Restoration of Erathia campaign. Now that's nothing compared to the expansion packs, much less to any high level 1v8 play or so on, but it's definitely not easy. Especially for me who has a very hard time with games like this.
I'm not much of a high-level achievement player I have to admit. While I do like to play some games for score and so on, I have a tendency to want to conquer something hard and then move onto another game. Though I do have a lot of games I like enough to want to improve my score in or do other challenging stuff in.

LMAO I remember in Mafia (1, NONREMAKE) there was this mission where it told you to chase a guy and kill him and the mission was designed so that it wasn't really possible to catch him, he was supposed to escape and the story would continue on, but I thought I HAD to catch him and would always restarted as soon as he got away without seeing what happens next, and eventually (after like several days of repeating the same mission over and over) I DID manage to catch him and kill him, and... nothing happened

The devs didn't expect anyone to actually kill the guy at that point.
Sadly I don't really enjoy playing games anymore but I used to be a tryhard fag in my teens, I always played and finished games on the hardest difficulty because I thought anything else was for pussies. In Trackmania I've completed E05-Endurance in under 1 hour and I got in top 10 worldwide times on official Nadeo maps on two occasions I think, in Warcraft III I've beaten 3 ALLIED insane computers solo, I finished The Witness while only using pen and paper on a single puzzle, in Lix I'm currently at 33 out of 40 extremely difficult Hopeless maps completed, in Xonotic I was the top damage player on the main CA server at one point I think.
EDIT: On the other hand I used to be a huge pokemon fanboy but I never actually trained any pokemon to level 100

I mentioned this in my introduction but my greatest achievement is beating I Wanna Kill The Kamilia 3, which I'd consider maybe the hardest or one of the hardest games of all time if you're not being pedantic and bringing up games that no one plays. You can watch my reaction to beating it in
this video. Although ironically the final boss isn't the hardest one in the game, the hardest one (in my opinion) is Piano which you can watch me beat in
this video. It took me 59 hours to beat. The whole game took me 473 hours although I did some unnecessary stuff like getting 80% of the crystals and doing the secrets (minus Secret 5 or any of the secret bosses).
Some other "I Wanna Be The Guy Fangames" that are somewhat hard I have beaten include I Wanna Be The Boshy, I Wanna Kill The Kamilia 2 (which is much easier than 3 but still hard), and I Wanna Lap Around which I consider a nice achievement because it's the hardest pure platforming-focused fangame I've beaten. It's also masterfully designed but hard to appreciate if you aren't autistic about fangames (and also probably impossible to beat).
And of course another achievement is maxing on Old School Runescape, which isn't really that hard and just takes a lot of time (~2000 hours). My bigger achievement in OSRS is getting an infernal cape which is extremely hard compared to anything else in the game and takes a lot of time, practice and dedication. I also got a quiver (you get it from the Colosseum which is a similar mechanical challenge to the Inferno) but it's easier IMO and also a full run takes way less time. Although those aren't really close to the hardest achievements in the game. I have a friend who achieved Grandmaster Tier and completed all Combat Achievements which includes basically doing the hardest challenges under certain restrictions so mine aren't really that impressive when compared. Although I am close to Master Tier but GM is basically impossible unless you get help from certain people on Discord lol. And some specific tasks are just too annoying to bother with.

I also beat Myst without a walkthrough on an old stream but I gave up on Riven. You basically need to spend a couple weeks with that game and write everything down, fuck that.

I'm ranked 7th (technically 6th but there's a bullshit technicality that makes me not consider that placement valid) in the world on a Mario Kart Wii track, DS Peach Gardens. The run is very outdated and almost 2 years old now, and the leaderboard is also very close together so I've failed 2nd place (1:58.986) paces countless times by now. Truthfully I'm at a bit of a mental block with this track, I've improved a lot as a player since I set this so I'm not the proudest of the run anymore, though it was insanely good for my skill level at the time I set it. Here is a link if you wish to see it.
This track is notable for the fact that automatic drift is very good at it due to the hedgemaze section, where unlike manual automatic can hold one wheelie through the entire way and it saves a bunch of time. This has resulted in auto, which is usually always inferior to manual, having held several world records on the course in the past. It is also tricky though, since at high levels you have to squeeze by the hedges while also hitting two frame perfect inputs during the wheelie resets (known as chain wheelies).
I'm also ranked
top 50ish on Moonview Highway, another track that automatic used to hold the world record on but no longer does, and I have a few other automatic times that are really optimised for the playstyle and that I am quite proud of but they don't rank as high due to auto being less viable. Most notably my
MMM and
KC times which I set pretty recently, both took long grinds and are the automatic best known times by a fairly large margin. As for why I'm so stubborn about using automatic? I guess it's a mixture of it coming more naturally to me than manual does and the fact that it's a somewhat old timey playstyle that I associate with nostalgically watching 2010 world record videos of when I was younger.

In Sanctum 2, which is a tower defense and FPS game, you're supposed to make use of both towers and first man combat, but I and another guy beat every map without towers and one of the maps took us a fucking week. Vidya of that map is
here (a lot of bug abusing was used, and if you also don't understand how weapons and abilities work then it probably looks unimpressive ^_^).
